What to do when someone dies
A Simple Guide

When a Death is Unexpected or Sudden

1. Call 999 – emergency services will attend.

2. Police may be present if the death is unexplained.

3. The death will be referred to the Coroner.

4. The person will be taken into the hospital mortuary.

5. The Coroner may order a post-mortem or inquest to establish the cause of death.

6. Once complete, you will be informed when you can register the death and arrange the funeral.

The Medical Examiner is not involved when the Coroner takes over.

When Someone Dies at Home or in a Care Home (Expected Death)

1. Call the GP or the out-of-hours doctor.

2. A healthcare professional will attend to verify the death.

3. The GP will refer the death to the Medical Examiner service.

4. A Medical Examiner will contact the next of kin to:

  • Confirm details about the person’s health

  • Review and approve the cause of death

5. Once approved, the Medical Certificate of Cause of Death (MCCD) will be sent to the local Registrar.

6. You can then register the death and begin funeral arrangements.

How to Register a Death

You’ll need to contact the local Register Office where the death occurred.

You'll receive:

  • Death Certificate

  • Certificate for Burial or Cremation (green form 9 and white form part D – often emailed to us but some registrars may give you the physical copy to pass on to us.)

  • Access to ‘Tell Us Once’ (to notify all governmental organisations)

Who Are the Medical Examiners?

Medical Examiners are senior doctors who review all non-coronial deaths in England.

They ensure the cause of death is accurate and will always speak with the family to:

  • Explain the process

  • Answer your questions

  • Ensure no concerns are left unheard

You don’t need to request this — it happens automatically.
